Log into the patient portal

As a patient, you can login to the patient portal to communicate securely about appointments, forms, financial balances, and more.

1. Open Google Chrome
2. Load the clinic's login page
3. Select Username or Chart Number and enter your username
4. Select Password and enter your password
5. Select Login

After you log in, select the menu options on the left to access the tools and information the clinic has shared with you.

Forgot username or password?

If you forget your username or password, select the Forgot username? or Forgot password? options on the login screen and follow the prompts.

If the forgot tools don't work for you, or if there's an issue with your contact methods on file, contact your clinic or administrator.
